Assuming proper C99 language support, we can select "the int type as big as
a pointer" (most of the time) or "the 64-bit int type" (nanboxing with
REPR_D), and then define everything else automatically.
This simplifies port configuration files. And the types can still be
overridden if needed.

When building the embedded port on MinGW-w64, I receive the following
error:
fatal error: alloca.h: No such file or directory
MinGW-w64 (used on MSYS2) doesn't include `alloca.h`, but `alloca()` is
provided via `malloc.h` instead. And this fix is also needed for other
Windows build systems.

Allows bytecode itself to be used instead of an mp_raw_code_t in the simple
and common cases of a bytecode function without any children.
This can be used to further reduce frozen code size, and has the potential
to optimise other areas like importing.

Obtaining the stack-top via a few function calls may yield a pointer which
is too deep within the stack. So require the user to obtain it from a
higher level (or via some other means).

Updates any includes, and references from Makefiles/CMake.
This essentially reverts what was done long ago in commit
